Labor of Love Home Health Services offers premier home health care that includes skilled nursing, home health aide, case management, occupational therapy, speech therapy, physical therapy & personal care attendant. We also provide private-pay services which include assessment, planning, resource linkage, reporting and professional services.
All services will be planned, coordinated and ordered under the direction of the approved treating physician through a well thought out plan of care appropriate to the approved needs of the consumer and under the supervision and oversight of an agency RN-Case Manager. Realizing that situations change, our services are flexible to meet your loved ones needs. The following services are provided when ordered by the approved treating physician as deemed necessary and appropriate to meeting the needs of the consumer and must be accompanied by a Doctors Order/Plan of Care. With the exception of Case Management, it is the physicians’ sole discretion to determine the frequency/need of either one or a combination of the unique services available.
This service may only be provided by a Registered Nurse, and is a mandatory service for all consumers receiving care. This service will automatically be requested from the approved treating physician by the provider agency on the initial Plan of Care. This service ensures quality assurance compliance, training, and coordination of care and advocacy for the consumer. In addition this provider will complete all re-certification documentation for continued every six months in coordination with the approved treating physician and after hour’s on-call coverage.
This service can be provided by an RN or LVN/LPN with training and expertise in provision of home care services. The nurse and physician are in constant communication to ensure the Plan of Care is appropriate to the needs of the consumer. Incorporated services may include but are not limited to; ongoing evaluation of patient needs, performance of skilled nursing procedures in accordance with the board of nursing standards and regulations as they relate to Home Health Care, education of patient, applicable family members and all direct support staff, self care techniques and prevention strategies, & coordination and continuum of care involving all inter-disciplinary health care team members.
Direct Support Staff- (Caregivers-Home Health Aides-Certified Nurses Assistants)
This service is to promote, encourage and assist with Activities of Daily Living, Mobility Assistance, Respiratory Activities, Companionship/Mental Stimulation,
Support Services and to comply with Specific Diagnosis Assistance strategies.
These services are delivered under the supervision and oversight of a Registered Nurse. The hours assigned may vary depending on the Physicians Plan of Care and in extreme cases, may even reach 24 hours of care.
